Kolkata : Fortis Hospital & Kidney Institute, Rash Behari, Kolkata has successfully treated a 76-year-old man with an unusually large prostate, weighing 290 grams – largest size prostate ever reported in Kolkata as per the available medical literature.
A team of doctors led by Dr. Shrinivas Narayan, Director, Urology, Fortis Hospital & Kidney Institute, Rash Behari performed a Holmium Laser Enucleation of the Prostate (HoLEP), a minimally invasive surgery that precisely removes the enlarged prostate gland, with minimal damage to the intricate surrounding areas.
The procedure was completed in just 1 hour 15 minutes, and the patient was discharged in a stable condition within two days.Upon admission, the patient was experiencing severe urine retention, a condition in which the bladder cannot be fully emptied, necessitating the use of a catheter.
A USG and CT scan were conducted to identify the cause of urine obstruction, which revealed an enlarged prostate – nearly the size of an apple.
Normally, such cases are treated with open surgery, which involves higher risks for elderly patients such as excessive bleeding and postoperative pain or discomfort. Considering these challenges, HoLEP method was chosen for its minimally invasive nature.

Typically, the average prostate size in elderly men is around 20 grams, but this patient’s prostate weighed 290 grams, which was a major concern and required immediate surgical intervention, as severe urine retention could lead to kidney or bladder damage. The patient’s age and history of hypertension made this case even more complex. Open surgery poses significant risks, including bleeding and infection, along with complications from general anesthesia, which can cause cardiac or respiratory issues. Considering these factors, HoLEP laser surgery was chosen and the enlarged gland was removed.
